Personalised medication packs
Each week we prepare your medicines in a sealed pack, with a compartment for each day and each time of day. This makes it easier to take every medicine at the right time.
Who is it for?
For people on long-term treatment with several medicines taken by mouth who find them hard to organise, for example:
- Because the schedule is complicated or the boxes are easy to mix up.
- Because of mild memory, sight or dexterity problems.
- Because they need help from a relative or carer to take their medicines.
It can be requested by the person, their carer or their doctor.
Which medicines are included?
Tablets and capsules taken by mouth. Some medicines cannot be taken out of their original packaging, so we keep supplying them separately:
- Syrups, drops, inhalers, eye drops, creams and injections.
- Medicines that must be kept in the fridge.
- Effervescent and orodispersible tablets, and others that deteriorate outside their blister.
- Medicines taken only when needed, unless your doctor sets a fixed schedule.
How does it work?
- We talk to you. In our private consultation room we explain the service, show you a sample pack and review your whole treatment.
- You give your consent. You sign the consent form for the service and we let your doctor know that you have joined it.
- We prepare your pack every week. Before it is handed over, a different pharmacist from the one who prepared it checks every compartment against your treatment.
- You collect it at the pharmacy. Each pack has a label listing the medicines it contains. The first time, we check with you that you can open it and recognise each dose.
- We keep it up to date. If your doctor changes your treatment, let us know and we will adapt the pack. When you collect a new pack, please bring back the previous week's one. You can stop the service whenever you wish.
